If you are serving the Insurance Commissioner / Department of Financial Services, the following rules apply:

The Department of Financial Services has several divisions. Please contact them at (850) 413-3114 for proper addressing of your service of process.
The original summons, writ, or subpoena, signed and/or sealed by the clerk of court.
Two copies of the summons, writ or subpoena.
Two copies of the complaint or petition (if applicable) and any attachments.
If you are serving the Insurance Commissioner / Department of Financial Services as agent for any type of governmental agency, no additional fee is required. If you are serving them as agent for an insurance company or other privately-owned business, you will need to enclose a $15.00 check made payable to Service of Process. This fee is required by the Insurance Commissioner, Department of Financial Services and we will serve it along with your paperwork.
If you are serving the Secretary of State as an agent, you will need to enclose a $8.75 check made payable to the Secretary of State. We will serve this check along with your paperwork.
